These Terms of Service (the "Terms") govern your use of services provided by Synergy Health Network ("Synergy", "we", "us"). By creating an account, enrolling in a membership, or using the platform, you agree to these Terms.
Memberships and Direct Primary Care Services
Synergy provides Direct Primary Care (DPC) and telehealth services through licensed clinicians. Membership entitles you to the services described in your specific plan. Synergy is not health insurance; we do not file claims with insurance companies on your behalf, and our membership is not a substitute for coverage required by federal or state law.
Eligibility
You must be 18 or older to enroll yourself, and a parent or legal guardian must enroll any minor dependents. You must reside in a U.S. state where Synergy operates. You agree to provide accurate, current information and to keep it up to date.
Billing and Cancellation
- Memberships renew automatically each month or year (depending on the cadence selected at signup) until canceled.
- You may cancel at any time from your patient portal or by contacting us. Cancellation stops future renewals; charges for the current period are not prorated.
- Failed payments may result in suspension of access until the balance is resolved.
Telehealth and Clinical Decisions
All clinical decisions are made by licensed clinicians at their professional discretion. Use of Synergy does not establish a doctor-patient relationship until a clinician has evaluated you. Synergy clinicians may decline to provide a medication, referral, or treatment when not clinically appropriate.
Acceptable Use
You agree not to (a) use Synergy for fraudulent purposes, (b) request prescriptions for someone other than yourself or your enrolled dependents, (c) interfere with or disrupt the platform, or (d) circumvent security measures. We may suspend or terminate accounts that violate these rules.
Privacy and Health Information
Your health information is protected under HIPAA. Please review our Privacy Policy and HIPAA Notice of Privacy Practices for details on how we use and disclose your information.
SMS Communications
By providing your phone number you consent to receive text messages from Synergy related to your care, appointments, prescriptions, billing, and platform-driven features (including AI-assisted intake). Reply STOP at any time to opt out. Standard message and data rates may apply.
Disclaimers and Limitation of Liability
Synergy's services are provided "as is" without warranties of any kind. To the fullest extent permitted by law, Synergy is not liable for indirect, incidental, special, or consequential damages arising from your use of the platform.
Changes to These Terms
We may update these Terms from time to time. We will post the current version on this page with an updated effective date. Continued use after changes constitutes acceptance.
